Permission and Release Forms
The Archdiocese of Detroit requires a completed Permission and Release form be submitted for each participant attending any off-property event or trip. Please download and print the appropriate form (Youth or Adult) using the links below, have each participant complete and sign, and bring the completed forms with you to Holy Fire. Group Leaders will be required to turn in copies at Will Call before your wristbands are released.
Safe Environment Clearance Verification Form
All adults (including parents) attending Holy Fire Great Lakes must be cleared by their home diocese, and submit a Safe Environment Verification Form signed by their Diocesan or Parish/School Safe Environment Official. This form will ask you to certify that all adults attending have been background checked and cleared in accordance with the policies of our diocese under the "Charter for the Protection of Children and Young People" as set forth by the United States Conference of Catholic Bishops.
Safe Environment Clearance Verification Forms must be submitted in advance. For groups, the Group Leader should submit. If attending as an individual, you will be responsible for submitting. Forms must be signed by your Parish/Diocesan Safe Environment Officer verifying that the adults listed on the form are compliant. Any adult not listed on a form will not be permitted to attend.
Submit the completed form to ProtectChildren@aod.org no later than October 5, 2022.
